Harris “Jake” Thomas Phylow, 74, of 166 Apple Tree Lane, Westminster, passed away on Saturday, October 6, 2012 at Heritage Healthcare of Toccoa.
Graveside memorial services will be held Monday, October 8, 2012 at the Calvary Church of God Cemetery.
